As part of submitting this application, you are agreeing that Lifebridge for Animals is offering low-cost spay/neuter to you for your dog with the following understanding:

• We pay ONLY for the cost of the spay/neuter and a 1-year rabies vaccination.  We are NOT responsible for additional charges such as flea/tick treatment, pain medication, of if an animal is in heat/pregnant at the time of the surgery.
• YOU are responsible for making the appointment and for arriving to the appointment on time. If you cannot keep your appointment, please call the vet’s office immediately to cancel and/or reschedule. The vet’s office reserves the right to return vouchers to us due to any no-call, no-shows.
• Be sure to follow all instructions given to you by the veterinary team before and after surgery.
• We are NOT responsible for any risks that may be involved with the surgery.
